You can do this by heading to Settings under near the user avatar on the top right.

 Scroll to the Contracts boxsection, head to the "Contract TemplateTemplates"  in order to create a questionnaire.

You can add, edit, hide in Client Portal or delete Contract templates.

There are some required fields when adding new contract templates: 

• Template Name: Enter the template name
• Type: A customizable drop-down list to choose the type of the contract template 
• Upload: Upload the contract template with the variables to be filled by the user once generated new contract.

The template can have multiple pages, groups, and variables. (these are the variables that will change whenever a new contract is created from this template)

Enter the page title, the group title, the variable name, the question, the type, the mapped-to field, and if it is required or not.

When setting up the contract's variable you need to set 4 mandatory fields:
